How to refer:
Health care providers can click the button to complete the Swallow Science Melbourne referral form via Halaxy.
Alternatively, please write a detailed referral letter and attach relevant results/information via email. If referring for VFSS, an imaging slip completed by a physician is required.
Chronic Disease Management Plan (GP)
GPs can refer a patient with a chronic medical condition and complex care needs for five Medicare rebateable allied health services under the Chronic Disease Management (CDM) Medicare items.
If the patient has already been referred by a specialist to Swallow Science Melbourne, please email the completed GP CDM plan to admin@swallowscience.com.au.
If you are the referring doctor, please upload the CDM plan to the Halaxy referral form. Alternatively, upload the CDM plan and a referral letter via email.
