For Referrers
Swallow Science Melbourne accepts referrals for adults with swallowing difficulties, including referrals from GPs, otolaryngologists (ENTs), respiratory physicians, gastroenterologists, neurologists, oncologists, geriatricians, allied health clinicians, support coordinators and other care providers.
Health care providers can click the button to complete a secure Swallow Science Melbourne online referral form, including for NDIS referrals. There is an option to upload relevant letters/documents.
Alternatively, please write a referral letter and attach relevant results or reports, then email it to admin@swallowscience.com.au
If referring for videofluoroscopic swallowing study, please complete and send a medical imaging slip (PDF available for download). This must be completed by a physician.
