If referring for videofluoroscopic swallowing study (VFSS), please download and complete the PDF imaging slip (physician referral required).
Chronic Disease Management Plan (for GPs)
GPs can refer a patient with a chronic medical condition and complex care needs for five Medicare rebateable allied health services under the Chronic Disease Management (CDM) Medicare items.

If the patient has already been referred by a specialist to Swallow Science Melbourne, please email the completed GP CDM plan to admin@swallowscience.com.au.
If you are the referring doctor, please upload the CDM plan to the Halaxy referral form. Alternatively, upload the CDM plan and a referral letter via email.
