Getting a Chronic Disease Management Plan (CDMP) from Your GP (FAQ)

If you’re considering an appointment at Swallow Science Melbourne, you might be wondering whether you can access a Chronic Disease Management Plan (CDMP) through your GP to help with costs. Below, we’ve answered some common questions to help guide you through the process.


What is a Chronic Disease Management Plan (CDMP)?

A CDMP is a type of Medicare plan that your GP can create if you have a chronic medical condition. It helps you access subsidised allied health services, like speech pathology, to manage that condition.


Who is eligible for a CDMP?

You may be eligible if you:

  • Have a chronic medical condition that has been (or is likely to be) present for six months or longer.

  • Have complex care needs that require treatment from a multidisciplinary team (your GP plus at least two other health professionals).


Examples of eligible conditions include (but are not limited to):

  • Progressive neurological conditions (e.g. Parkinson's disease, MND)

  • Stroke

  • Cancer

  • Chronic respiratory conditions

  • Other long-term swallowing or communication difficulties

If you’re unsure, we recommend speaking with your GP.


How many sessions are covered by Medicare?

Medicare currently covers up to 5 allied health sessions per calendar year under a CDMP. These sessions can be split across different providers (e.g. speech pathology, physiotherapy), so it's worth planning how to use them. You may only need one or two sessions with us, so keep that in mind.


What do I need to bring to my appointment?

To claim a Medicare rebate, we need a valid referral from your GP using the CDMP format (often called an “Allied Health Referral under Medicare – Individual Services under a CDM Plan”). Please ensure:

  • Your GP has listed Swallow Science Melbourne (or your speech pathologist’s name) on the referral.

  • The referral is dated on or before the day of your appointment.

  • You bring a copy of the referral (or ask your GP clinic to email/upload it directly to us).


Do I need a new referral every year?

Yes, a new referral is generally required each calendar year if you need more sessions. This also allows your GP to review your ongoing needs and progress.


How do I claim the Medicare rebate?

We provide you with a detailed invoice after each session. You can then submit your claim to Medicare:

  • Via the Medicare app or myGov portal

  • By visiting a Medicare service centre

We do not offer HICAPS or on-the-spot claiming at this time.


How much will I get back from Medicare?

The current Medicare rebate for a CDMP session with a speech pathologist is $61.80 per session (as of 16/09/2025). The rebate increases significantly if you have reached the Medicare Safety Net. Please note:

  • You will still have an out-of-pocket cost, as our fees are higher than the rebate amount.

  • You are not able to claim from both Medicare and your private health fund for the same session.


How do I organise a plan with my GP?

  • Book a longer appointment with your GP.

  • Ask whether you’re eligible for a Chronic Disease Management Plan.

  • Let them know you'd like to be referred to Swallow Science Melbourne for speech pathology services.

  • Your GP will either complete the referral during the appointment or schedule a follow-up to finalise the paperwork.
