Flexible Endoscopic Evaluation of Swallowing (FEES)

A Flexible Endoscopic Evaluation of Swallowing (FEES) is a procedure used by speech-language pathologists and ENTs to assess swallowing function. During a FEES, a thin, flexible camera is passed through the nose and into the throat to provide a clear view of swallowing in real time. This allows healthcare professionals like speech pathologists and ENTs to observe any swallowing abnormalities, and determine the cause of swallowing difficulties. FEES is a valuable tool in diagnosing and managing swallowing disorders, guiding treatment plans, and improving patient outcomes.

FAQs:

How long does a FEES procedure take?

Within a standard 45-minute initial appointment, the FEES assessment can last anywhere from 10 to 20 minutes, depending on what needs to be assessed and how long the person tolerates having the camera in the nose.

I'm nervous about having a camera down my nose and throat! Can you give me something to numb it?

We don’t usually use numbing sprays for FEES because they can reduce normal throat sensation, which is an important part of swallowing. Some people find that self-administering their usual topical nasal decongestant spray before the appointment (which reduces swelling in the nasal passages) makes the scope feel more comfortable as it passes through. Swallow Science Melbourne cannot recommend, supply or administer medicated sprays, so please speak to your referring doctor or pharmacist before your appointment to get tailored advice for your situation. FEES can be generally well-tolerated without any sprays. Click here if you would like more information on this topic.

Is FEES safe? What are the risks?

Yes, FEES is considered a very safe procedure. Complications are rare, but may include:

  • Nosebleeding (epistaxis): up to 1.2% of cases

  • Vasovagal episode (fainting): up to 0.1%

  • Laryngospasm (brief, involuntary closure of the vocal cords): up to 0.07%

Most people have no complications, and any that occur are usually mild and short-lived and can be managed in-clinic.

What are the benefits of FEES?

FEES gives a clear, real-time view of your throat while you swallow, allowing your clinician and referring team to:

  • Identify swallowing problems accurately, including silent aspiration (when food or liquid enters the airway without triggering a cough)

  • Determine the most suitable food and drink textures for safe swallowing

  • Show you your swallow, helping you better understand what is happening

  • Trial techniques and strategies to improve safety and ease of swallowing, reducing the risk of chest infections, malnutrition, and unintended weight loss

Will I be able to eat or drink after the test?

Yes, most people can eat and drink normally straight after FEES. If the assessment shows changes are needed for safety reasons, these will be explained to you before you leave or once the images are reviewed in more detail.

What should I bring to my appointment?

We will have a variety of foods and drinks available for testing, but you are welcome to bring:

  • 'Problem' foods or drinks you commonly consume that cause swallowing difficulty

  • Any special/adaptive cutlery you use for eating or drinking

This helps us assess swallowing with items you are familiar with and may wish to continue using.

Can someone come with me?

Yes, you may bring one support person/carer. They may be asked to wait outside during the procedure due to space or infection control measures, but can join you for the discussion afterwards.

Will I get my results straight away?

In most cases, yes. We will explain what we saw and may show you some aspects of the recording. The images will be reviewed again in detail, and a written report will also be sent to your referring doctor and/or local speech pathologist.
